Order a New Prescription

When your physician writes a new prescription, complete the Adobe PDF CarolinaCARE Prescription Order Form. On the order form, make sure to complete all required information, including name, member ID, shipping method, method of payment, and who the prescription is for.

You can then:

  • Send via U.S. mail (please do not use interoffice courier) the completed order form and your prescription to:

    Pre-addressed CarolinaCARE mailing envelopes are available at regional Human Resources centers, the Medical Center Plaza (MCP) Pharmacy, the CMC North Cross Pharmacy, and CarolinaCARE.

A Payment Method is required before an order can be processed.

Please allow 10-14 days for delivery of prescriptions by mail. Without a method of payment, mail order prescriptions cannot be delivered.

North Carolina Board of Pharmacy Regulation (Rule .1806) specifically:

  1. Requires that prescriptions can only be faxed from the original prescriber's office.
  2. Prohibits employees (patients) faxing their own prescriptions.
  3. Physicians should not prescribe for themselves and their family members.
